Publisher information is contained in the <publisher>
element, with the name of the publisher in the <publishername>
element and the address of the publisher in the optional <address>
element.
<publishername>
can also be the name of a web site or other, less formal, publisher.
For some resources, for example personal communications, publisher can be omitted.
The <address>
element can be used with plain text if the address is simply the city, city plus state/province, or city plus country. I recommend that if it goes beyond this simple use (or even if it is that simple), that the child elements be used. This is particularly helpful if the markup contains a full street address, since styles typically don’t use street addresses. Using the child elements lets the processor decide how much of the address to use. See Open Question 8.
